ENTER NAME AND QUANTITY OF SPEED SKATING SESSIONS

IN CONSIDERATION of my involvement in the sport, activities, and events under the auspices of U.S.
Speedskating and/or a USS Association and/or a USS Club, I acknowledge, appreciate, and agree that:
1. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS ASSOCIATED WITH THE SPORT OF
SPEEDSKATING; both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE
RELEASEES OR OTHERS;
2. THROUGH MY PARTICIPATION IN ANY SPEEDSKATING EVENT, I RISK BODILY INJURY, (INCLUDING
PARALYSIS, DISMEMEBERMENT, DISABILITY AND DEATH), SICKNESS AND DISEASE (INCLUDING
COMMUNICABLE DISEASES), AND EXPOSURE TO ADVERSE CONDITIONS (INCLUDING ISSUES WITH
FACILITIES AND EQUIPMENT); and while particular rules of my sport, equipment, and persona training and
discipline may reduce this risk THIS RISK OF INJURY DOES EXIST, AS WELL AS THE RISK OF DAMAGE TO
OR LOSS OF PROPERTY;
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe
any unusual or unnecessary hazard during my presence or participation, I will bring such to the attention of the nearest
official; and,
4. I, FOR MYSELF, AND ON BEHALF OF MY HEIRS, ASSIGNS, PERSONAL REPRESENTATIVES, and NEXT
OF KIN, HEREBY RELEASE, HOLD HARMLESS, and PROMISE NOT TO SUE US SPEEDSKATING, USS
ASSOCIATIONS, USS CLUBS, AND THEIR UTILIZED VENUES. THE INTERNATIONAL OLYMPIC
COMMITTEE, THE UNITED STATES OLYMPIC COMMITTEE, THE INTERNATIONAL SKATING UNION OR
OTHER SPONSORING ORGANZIATIONS, THEIR OFFICALS, VOLUNTEERS, STAFF, SPONSORSORS
AND/OR AGENTS (“RELEASEES”) WITH RESPECT TO ANY AND ALL INJURY AND LOSS ARISING FROM
MY PARTICIPATION, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE,
EXCEPT THAT WHICH IS THE RESULT OF GROSS NEGLIENCE OR WANTON MISCONDUCT.
5. GOVERNING LAW: This Agreement, and its validity, interpretation and construction (weather in contract, tort or
statue), or the negotiation, execution or performance of this Agreement (including any claim or cause of action based
upon, arising out of or related to any representation or warranty made in connection with this Agreement, or as an
inducement to enter into this Agreement), shall be governed by, and enforced in accordance with the internal laws of
the State of Utah.
By completing the registration for this event, I acknowledge that I have read this Release of
Liability and Waiver Agreement and fully understand and agree to its terms.

This is to certify that I/we as parent(s)/guardian(s) with legal responsibility for this participant, do consent and
agree not only to his/her release, but also for myself/ourselves, and my/our heirs, assigns and next of kin to release
and indemnify the Releasees from any and all Liability inciden

In consideration of being permitted to participate in any way in the Wolverine Sport Club (WSC) sponsored bicycling, speedskating, cross country skiing and other activities (Activity), I, for myself, my personal representatives, assigns, heirs, and next of kin:

Acknowledge, agree, and represent that I understand the nature of WSC Activities and that I am qualified, in good health and proper physical condition to participate in such Activity. I further acknowledge that the Activity may be conducted over public roads and other facilities open to the public during the Activity and upon the hazards of which traveling are to be expected. I further agree and warrant that if at any time I believe the conditions to be unsafe, I will immediately discontinue further participation in the Activity.

Fully understand that: (a) bicycling, speedskating, cross country skiing, and other WSC Activities involve risks and dangers of serious injury, including permanent disability, paralysis and death (Risks); (b) these Risks and dangers may be caused by my own actions, or inactions, or the actions or inactions of others participating in the Activity, the condition in which the Activity takes place, or the negligence of the “Releasees” names below; (c) there may be other risks and social and economic losses either not known to me or not readily foreseeable at the time, and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the Activity.

Hereby release discharge, and covenant not to sue the Club, their respective administrators, directors, officers, volunteers, any sponsors, advertisers, and if applicable, owners, and lessons of premises on which the Activity takes place (each considered one of the “Releasees” herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be causes in whole or in part by the negligence of the “Releasees” or otherwise; and I further agree that if, despite this Waiver of Liability, Assumption of Risk, and Indemnity Agreement I, or anyone on my behalf, makes a claim against any of the “Releasees”, I will indemnify, save , and hold harmless each of the “Releasees” from any litigation expenses, attorney fees, loss, liability, damage, or cost which may occur as a result of such claim.

I further agree to allow all photographs, video and/or any digital images reproduced in association with WSC activities to be used in any way by WSC, and release all claim to rights in and to those images. I permit the WSC or other Released Parties to re-use, publish, and republish photographs or pictures of me or in which I may be included, in whole or in part, or composite or distorted in character or form, without restriction.

I have read the agreement, fully understand its terms, understand that I have given up substantial rights by signing it and have signed it freely and without and inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extend allowed by law and agree that if any portion of this agreement is held invalid, the balance notwithstanding shall continue in full force and effect.
